Your First Visit to Our Wayne Practice

Tell Us A Little Bit About You
During your first visit to our office, you will be asked to fill in a few pages so that Dr. Tarlow can get an understanding of how she can help you. If you prefer, you can fill in the needed paperwork prior to your visit. You can download the forms under "What to Expect" then "Online Forms". If the visit is for you, please download and fill in the "Patient Introduction" form and the "Adult Consent" form. If it is for your child, please download and fill in the "Patient Introduction Form" and the "Child Consent Form". You should allow an hour for this visit. You should allow an hour for this visit if your paperwork is filled out ahead of time.
Chiropractic Consultation and Examination
Then, you will have a consultation and chiropractic examination so that Dr. Tarlow might further understand the nature of your health problem. She will most likely want to know the following information:
- When did your (or your child's) problem begin? What was happening at that time in your life during the onset?
- What factors make your (or your child's) problem better or worse? Is the problem constant or does it come and go?
- Have you (or your child) had any adverse reactions to any medications or injections?
- If the appointment is for your child, and he (or she) has behavioral issues (ADD, ADHD, ASD or dyslexia, head banging or defiance) or chronic health problems (ear infections, frequent colds, bronchial/sinus infections, seizures, tics, etc.), Dr. Tarlow will also want to have information about your child's diet, traumas (physical or emotional) and the child's birth. If your child has severe behavioral problems, it is best to have a gluten-free, dairy-free and chemical-free (sugars and dyes) for 24 hours prior to the first visit.

Chiropractic Adjustment and Follow Up Visit Instructions
Most likely, you or your child will receive an adjustment (with your consent of course) and given instructions when you should return for a follow-up visit.
You will be asked to schedule a follow up visit. Payment can be made in either cash, check or credit card.
